FREQUENTLY ASKED QUESTIONS

Listed below are a series of commonly asked questions from patients when dealing with our practice and facility. After reviewing these, if you need additional information, feel free to call us at  203.407.3500.

Is there a physician on call after hours?
Yes, we are on call 24 hours a day, 7 days a week. Call our main number, 203.407.3500, for assistance. Please note that prescriptions are refilled during office hours only.
Will I need a referral to schedule an appointment?
We generally do not require a physician referral to our office for the purpose of scheduling an appointment with one of our physicians, unless this is a requirement by your insurance carrier.
Are you seeing new patients and what age range do you see?
We are currently accepting new patients and we treat all ages.
What is your policy for releasing records?
We have release form s in each office; otherwise you can write in a request that includes your personal information as well as information on where records are to be sent.
Is parking available and free at all of your locations?
Yes. In New Haven, we will validate your parking stub.
What hospitals are you affiliated with?
St. Raphael’s Hospital and Yale New Haven Hospital.
How are emergencies handled after hours?
If you have a medical emergency, call 911 or go to the nearest hospital. Be sure to give the hospital the name of your treating physician so we can be contacted directly. All other non-emergency calls for existing patients are handled by our on-call physician at 203.407.3500
Do I need to bring my X-rays if they are negative?
Yes, all X-rays/MRI/ct-scans on CD or film that are pertinent to the part of body being seen, need to be brought in with the patient.
Can my minor child com to the appointment alone?
All minors need to be accompanied by an adult.
Why do I have co-pays?
Your co-pays are part of a contract between you and your insurance carrier that we honor.
How do I refill my prescription?
You may call your doctors assistant or you may call your pharmacy and they will request a refill from your doctor.
May I return durable medical equipment?
Please be aware that due to OSHA (Occupational Safety and Health Administration) regulations, we not able to accept returns of any durable medical product.
Are the doctors board certified and what are their credentials?
Please refer to each physicians page to better understand your doctors credentials.
Do you have a wheelchair and are you handicap accessible?

Why do I have to fill in forms again if I am already a patient?
They need to be updated once per year for any changes or you have a new injury.
Why do you need to copy my license?
This is a new federal requirement due to insurance fraud and identity theft.
